2. I request for a Debit Card with POS and ATM services and Personal Identification Number (PIN) under the Terms and Conditions which can be found on the reverse of this application form

3. I certify the information given to be accurate and true. I agree that the Pentecostal Community Bank may make such enquiries about me as it considers necessary in relation to this application
